In the text boxes below, you can write in response to the prompts. When you submit the result it's private: YOU will get a copy of the text and you can over time modify that text so your Personal Health Plan and what you do in your life to be more healthy, reflect each other. You then have a tool to measure you own progress and discuss with you medical advisers your health coach and your fitness coach.
This is a way to take control of your life. No magical process. Understand what you want to do, and slowly develop a way of living your life so the Better Health is Achieved.
If you are under 65 and you retain some of your youthful health, make a resolution NOW to do two things.
Get your diet right! Failure to do that will undermine everything else you try to do.
Don't let your fitness and strength slip away. Exercise with some intensity, at least 3 hours a week.

Here is a link to a PDF page where you can begin to set out some health priorities for yourself. This is not about being perfect, it's about making a start. Your Personal Health Wheel. You are the ONLY person qualified to make these decisions. Your own thinking and committment is the secret.
If you've already dropped that ball, if you are older and perhaps retired: but now weak, overweight, with joint pain, probably with metabolic syndrome and type two diabetes, you are actively switching off your own future. Turn it BACK ON, but you have to DO SOMETHING different. It's never too late. Diet First. Then exercise. (Later sleep and social engagement) If retired, you now have the TIME to become an Athletic Senior, slowly you need to TRAIN YOURSELF to be stronger, faster, more flexible, capable of doing most of the things you used to do, like running, dancing, or hiking with a pack, and cycling 50 or 60 km. Even just going to the gym or the pool on a regular basis.
This is not a "New Years Resolution." Meaningless. It's making a choice to be healthy and following that up with work. It doesn't demand any special medical or health expertise. Your own knowledge is the key to success.

You will get an email text that you can copy into Word or a text writer. When you finally print something to show your doctor or health coach, you might add any personal health records you have made at home, sleep hours, heart rates, blood pressure, blood glucose readings, ketone levels, anything else of interest.
There's no need for this form to check that you are human before you submit. There's nothing being sent to Open Future Health except an email note saying someone has used the form. Your email address remains private.
If you find your health plan helpful and you want to communicate with us, you can easily do so from the website. OR when you get your copy of the text you wrote above, you can reply to the sender address in that email.
